A complimentary “Meet-You” visit with Dr. Shaw is available for patients who are considering joining the practice. During these initial one-on-one sessions, we’ll discuss our practice philosophy, your expectations, as well as your family’s preferences and goals for your children’s health. We’ll then collaborate with you to decide which traditional and holistic approaches to illnesses will be the right fit for you and your family.

New Patient Forms and Insurance Information
At Touchpoint Pediatrics, we strive to offer a comprehensive approach to pediatric medical care, especially for our new pediatric patients. Before their first appointment, new pediatric patients and families are asked to complete a Patient Registration form for each child. If you’d like to transfer your child’s records from another practice, download our Transfer of Records form, complete it, and send it to your previous pediatrician – we’ll take care of the rest.
